Make a UAP Report
If you are a pilot, aircrew or aviation professional and you want to report an aviation-related
observation or incident involving UAP, unidentified aerial phenomena, please take all of the following
steps. NARCAP will protect the confidentiality of reporting witnesses.

Current Cases (occurring within the last two weeks):
If you are a pilot, aircrew or aviation professional and are reporting a current aviation-related
observation or incident (occurring within the last two weeks) please immediately send an email with
your contact information and the date, time, and location of the incident to Ted Roe at , then complete the steps below. There is a narrow window of time to request
supporting documentation from the FAA so we need to know about current cases immediately.

2. Contact the FAA Aviation Safety Reporting System and make a confidential report. It is appropriate
to report these incidents to the ASRS to enable safety administrators to consider these events within
the context of aviation safety.

To report an incident or situation to ASRS, download the appropriate Reporting Form from the
FORMS Page on the ASRS website, print, fill out and mail the completed form.
